Frequently Asked Questions

I am a Gold Star Surviving Family Member. What documents do I need to show proof of eligibility?

Respectfully we request:

Official Documentation:

A DD Form 1300, the Department of Defense's official Report of Casualty, is a primary piece of proof for a Gold Star family.

Additional Acceptable proof:

State-Issued Plates:

Many states offer special Gold Star license plates to eligible family members, which can be verified through the state's Department of Motor Vehicles or Veterans Affairs.

Gold Star Lapel Pins:

The Department of Defense issues a gold star lapel button to immediate family members, such as spouses, parents, and children, of service members killed in the line of duty.

Gold Star Service Flag:

A family may display a service flag with a gold star, which is a symbolic representation of the loss of a family member serving in the Armed Forces.

How do I provide proof?

I'm a disabled Veteran. What documents do I need to show proof of eligibility?

VA benefits letter.

How a Veteran Can Get Their Benefits Letter

Sign in to VA.gov: The veteran must sign in to their account at VA.gov.

Access VA Letters and Documents: They will then navigate to "VA letters and documents".

Find the Benefit Summary and Service Verification Letter: Within this section, they can find a letter that shows their current monthly award, their combined service-connected disability rating (which would be 100%), and confirm if they are considered "totally and permanently disabled"

Other Documents a Veteran Might Possess

A veteran might also show you other identification documents, but these do not confirm their 100% disabled status:

DD214: The form issued upon separation from the military.

Veterans Health Identification Card (VHIC): A card from the VA.

State-Issued ID: A driver's license or state ID with a "veteran" endorsement.

Disabled Veteran Plates or Placards: License plates or vehicle placards indicating disabled veteran status.

Are your forms secure?

Our forms encrypt data both in transit (TLS) and at rest (AES-256) on secure, cloud-based infrastructure.
